If you are a bona fide counselor or psychotherapist or other recognized practitioner and would like your own page in the therapyindex.com directory, simply complete the three sections of this form and click Submit. You will then transfer to the WorldPay secure payments secure server where you can enter your Credit or Debit Card details. We welcome Visa, Mastercard, JCB, Solo, Switch and Delta.

Your card will be billed £5.85 (or equivalent in the currency of your choice - US$, € euro, AU$ or NZ$) automatically each quarter, and you can cancel at any time.
